Transition to Adulthood (T2A)

Every year, more than 500 young people in Queensland transition out of the care system. These young people are in need of support from carers, friends, family and their community to ensure they have what it takes to live a prosperous and independent life.

Thrive House are identified by all relevant stakeholders as a leading preferred provider who have the capacity and commitment to ensure the smooth transition of child safety participants age 15 to 18 to either NDIS Access and or to semi / independence.

This service is available to all young people whether they are transitioning to independence, adulthood or SIL.

This is no additional expense, and is an intensive transition – developing life skills across  multiple domains; social, personal, legal, family, community, cultural, medical, daily living - provided by same core care team as they are familiar with. This enables young people to remain where there are during the transition to ensure they are comfortable and confident throughout this process.
